Have you configure the options in Admin CP > Options > Who has visited?

At the very least you need to chose which usergroups to show it to.

I've just tested this on my main site. It's related to the display limit you set in the ACP.

If the number of people who have been online in the time period set is lower than the limit you want to display, the end name is proceeded with a comma.

The very last item in the online recently block won't have a comma (as you would expect):

When you show the remaining items, it still doesn't have a comma (but that should be ok because the rest are on a new line) but the remaining ones are separated by commas until the last one:
