We merged two accounts, and on user cache rebuild this happens now:

You appear to be using a very out of date version, nearly 3 years old. Please update and see if the issue still occurs.

We updated as far as we can, to 2.5.4, the latest Warning Improvements require 2.2, which we'll be doing soon, so we're not there yet. I can update to see if it's resolved by then, but it doesn't seem we can do user cache rebuilds until then.
 
Warning Improvements 2.8.7 and Report Improvements 2.10.14 are the last versions of those add-ons to support XF2.1, which is more than enough to update Warning Acknowledgement to the latest version.

Yes, what I meant was we haven't updated to XF 2.2 yet so we just can't do those upgrades yet, we'll have to test this again at the later time when we've completed the scheduled upgrade.

Apologies I typo'ed the XF version number. Warning Improvements 2.8.7 and Report Improvements 2.10.14 support XenForo 2.1

Oh, I think what added to this misunderstanding Warning Improvements is downloaded directly here on Xenforo's resource, clicking download gives the latest version. We're not able to select 2.8.7.

Or is there a way we're just not seeing?
 
The pending warning action user is still allowed to view/post. Example:
