XF 2.2 Not receiving some alerts

Has anyone ever experienced an issue where they get some alerts but not others? For instance, sometimes people will tag me in a post and I'll get the alert, but other times I won't get it at all.

This is using XF 2.2. No errors in the log or anything either.
 
Months later and this issue still persists with no rhyme or reason. If someone tags someone else, half the time they'll get an alert, the other half they get nothing at all.

I can't be the only one experiencing this issue, can I?
 
We have had no other reports of it.

Are you able to reproduce it consistently with all add-ons disabled?
I’ve disabled all add-ons and it still happens. The issue is that it’s entirely spontaneous. If I tag 100 people in 100 separate posts, only one of the alerts will fail. It’s as if there is no real rhyme or reason.
 
I've also experienced this on my own XF site. Sometimes I don't get any alert for new content of threads I'm subscribed to, and which I've received an alert for previously.

On another XF forum I sometimes don't get new email alerts for new threads in subforums I'm subscribed to.

I was thinking a cause could be new threads/replies that require admin approval before being displayed, but I confirmed that wasn't the case for the most recent time it happened. The person replied a day after I had last viewed it and there were no other removed/approved comments between.

I don't have email alerts enabled for my own account on my forum (it's just the alert icon I use). I'll keep paying attention to this issue to see if I can notice particular things about it.
 
Ok, it just happened when an admin moved a post (the OP of a thread with no responses) to a thread I had previously created. The node showed bolded and the thread was bolded, but I received no alert as I usually would if someone replied to my thread.
