XF 1.4 Member added to conversation not either original member

I have a situation where two members are telling me they were in a conversation together and then randomly a new member was in the conversation. Neither claims to have invited them. How is this possible? Is there any way to see how this random member was added to the conversation?

If one of the existing members added them, an email should have been generated.

I can't think of any reason why that would happen otherwise, unless an add-on is involved.

Both members would get that email?
