It really depends on ISP's routing... I remember the major drop in Japan is owned by either NTT DOCOMO or whatever its called... so if you're using any other ISP, it'll also do some fun routings to get into NTT DOCOMO's network, and then out through the drop.
Oddly enough, the way it works here: NTT owns the fiber network and you pay them a small fee per-month to access it. When you order the main service, you then pick an actual ISP. So, NTT provides the fiber connection and ONU to interface with your home network. In my case, I chose OCN to provide the actual 'internet' service. The monthly bill is combined and comes to about 4,000yen (~$47.00USD).
