XF 1.5 Users Not Receiving Tag Alerts

I have many instances of users not receiving alerts when they are tagged in threads.
It seems to occur all over the place, and I'm not exactly sure what the deal is.

Also, does XF user tagging support the ability to tag a usergroup or something? For instance I have 30 people I want to tag, is there a way to do this with one tag that those 30 users fall under?

XenForo does not have group tagging, it can be added via add-ons.

Are you tagging more people that permitted to be alerted via your permissions?

It is the "Maximum mention alerts per message" permission under user/usergroup permissions. If set at 5, only the first 5 users mentioned will receive an alert for the mention.
