It needs one option to set the timer it checks for new alerts, it's friendly to load on bigger forums.

No, but I hope for it too.

Yes on user alert preferences users can chose to not hear the sound on new alert. As i found out some leave the site open while they do things they're focused on and get mad when it beeps.

In Tab Alerts v1.3 I introduced an option to have an audio alert. Unfortunately I overlooked that some users will have many tabs open to the same forum, this caused the audio beep to be heard many times.

Tab Alerts v1.4 removed the audio alert option.

I'd say create an option in the users preferences to enable/disable the audio?

But still if the user opens multiple tabs, audio would be annoying.

It happens with Facebook. You have multiple tabs open and you get an inbox, you here the notification sound on all tabs at once.

It's just for Inbox/Private message not for Alerts/Notifications.

I don't know if this is just me, but sometimes I realize that the actual notification icon doesn't show up when I get a new notification. I know I have one because of the tab alerts, but when I go to the tab, the default xenForo notification icon doesn't show until I refresh.

My guess is it's your style has the problem.
Try to use xenforo default style.
