Fixed Inline spoiler & screen readers

The [ispoiler] bb-code interacts poorly with screen readers, as it is a span without any indicators it has content under it for the reader to interact with.

Experimentation shows that tabindex="0" role="button" helps a number of screen reader extensions understand there is something there to be interacted with and to show the spoiler.
