If you mean for your main XF installation, it can be installed in the root or a directory, it makes no difference to the add-on.
If you want the home page to be the root domain though, XF should be installed in the root.

As far as installing the add-on is concerned, you just upload the contents of the upload folder (the same as with most other add-ons) and then import the xml file.

The direction is controlled via the language options, it's not related to the add-on.

You just need to set up the options and style properties accordingly.

I noticed on the CTA Featured Threads and Portal page the new posts don't show a number.

These screenshots were taken at the same time. The new posts indicator with a number is the forum itself. The no number one is CTA.

That indicator is from an add-on, its' not part of the default software so therefore won't work on the portal page.
