Is there a resource that details how alerts work?

We're getting a lot of complaints about alerts not working, or rather, people not getting alerts when they expect to.

What should I check to test this out? Which things affect the frequency of alerts?

How do alerts actually work? When does it check and send out new alerts? Sometimes you seem to get a new one for every post in a thread, other times just the earliest one appears with "there may be more posts after this".

(1) Alerts Expire. So if you don't log in regularly, you'll miss them.
(2) Alerts stop coming if you ignore them *ONCE*.
example: you are following a thread (watching) and you login to the site. There is an alert about the thread, but you are busy and don't read it.
Now, you'll never be alerted ever again !
