1.1.2 - Insert Image - not showing correctly?

Strangely enough this works for media but not "insert/edit image" when quick replying

I'm not sure what could be doing this...

I saw this once before. I remember it was caused by an addon or template customization. You will need to disable addons and troubleshoot to isolate the problem.

I was hoping this wasnt the case, but a part of me felt it was... oh christ.

So in addition - I'm noticing extremely sluggish time to have the actual window appear for the quick-reply - I think the two are related..

Here's the before and after (in-between takes 2-5 secs to load)

The only real thing I changed was adding a lot more forums and adjusted the permissions for each. Could the permission checking to see if I can reply actually take that much longer to calculate?

Nope.

Nope.

It turns out this was cloudflare - I cleared the cache and the errors/issues vanished. Just wanted to update you.
