This add-on introduces two new Permissions in the Admin CP. A Forum permission to allow a user to add a poll to their own existing thread, and a Forum Moderator permission to allow a user to add a poll to any thread.
If a user has permission to do this, they will see this in the Thread Tools drop down:

Moderator:
Once clicked, the poll creation fields appear in an overlay:
And finally, the poll is created:
