Fixed Thread title edit duplicates moderator log

Tried this on XF demo with no add-ons and same result.

Steps to reproduce:

1. Inline edit first post of a thread made by a different user
2. Change the thread title and nothing else
3. Click on "Save"
4. Check the "Moderator actions" page for the thread

Only seeing a single entry here and in the moderator log in the ACP.

The first two were triggered by a single thread title edit.

Tried again in the same thread in the demo installation and the result:

It's possibly something which has already been fixed on this site.
