XF 1.3 Duplicate Post ID's - how to fix?

For some reasons we're seeing a number of threads where posts have the same ID. This is throwing the post chronology out.

Any idea why this is happening and what the fix is?

It shouldn't be happening as there is code that is there to prevent it. I couldn't rule out an add-on being involved if there's something interacting with the posting process.

It can be sorted by rebuilding thread information and rebuilding post positions.
