For every log in attempt? No. Only repeated failed attempts.

But why would bots be logging in successfully? They wouldn't. So a CAPTCHA that all users have to complete to log in would only really slow down genuine and annoy genuine users (even if it is one that's easy to solve).

Failed login attempts already trigger a captcha in an attempt to prevent brute force attacks if that option is enabled. The default, however, is better -- repeated failed attempts will have the user performing the attempts locked out completely.
