How do you determine how long a user has been online for?
We track session activity based on XenForo’s session system, recording active time when a user is interacting with the site. Idle or inactive time isn’t counted.

So if a user session is set to 1 hour and they only view one page load, does it count as an hour?

No, it only counts the actual active time when the user is interacting with the site. As noted in the feature list, inactive sessions are ignored.
 
It is quite similar to a component I am currently using in vBulletin and is a valuable feature.

I believe this should be a default feature for this add-on.
It is a fundamental requirement.
When tracking online time, we must be able to generate statistics based on daily, weekly, monthly, and yearly periods, as well as for any specified time range.

For example, is it included in the time spent in a add-on written specifically for me?

Yes, the add-on tracks time based on XenForo's session system. Since XenForo's session system tracks user activity across the forum, time spent in custom add-ons should also be counted, as long as they don't bypass XenForo's session handling.
