XF 1.5 Upload Attachment button on every post.

We're using UI.UX and we've haven't uploaded any addons or done any theme changes for a few months now.

However, all of a sudden this happened:

On every post, and the title of every thread is "Upload Attachments | <real title>"

Any idea why this is happening?
