ssh -p 100 user@your.ip.address (or domain.com)
Where 100 is the port # you have changed to.

For me, it's hust ssh -p xxxx domain.com since I'm using keys.

Terminal on Linux and Cygwin mintty on Windows. Always running screen everywhere.

+1000 for SecureCRT

I prefer Cygwin, a zillion times better than SecureCRT. You get all the Linux power in Windows.

OSX: I use Terminal, with the SSH command.
Windows: PuTTY, since it's what I used in my windows days and I don't know any better.
iOS: I have iSSH, but don't use it much, since the iPad (as a whole) is fairly dreadful as a CLI. I also plan to check out Prompt, since it was mentioned in this thread.
