Give the gray "Default" box a green or a red shade depending on which permission state is inherited and use e.g. a gray box with red border if it's 'No' due to not having any explicit value acting on the permission.

Something like this would make it easier to use and understand.

Do you mean in node permissions? In user group permissions, there's nothing to inherit from (so it's always no). In a user's permissions, I suppose we could look at what their groups give.
 
It would only work for users as usergroups are standalone.
It's only when a user is a member of multiple groups do cumulative permissions come into effect.

one could also interpret the op to mean "make it more clear that in some cases perms 'default' means 'no'".
i think they grey colour implies a neutrality that is, in most cases, misleading.
