Have you had the page open for a long time before attempting to submit the update?
Try opening the page in a new tab and trying again.
What size is the file?
If it's a large file and it exceeds the post_max_size value then it can present itself in this way.

Hmm, your post_max_size is 8M and your upload_max_filesize is 64M so it should be fine.
You could try increasing the post_max_size just in case.
You can do that by editing the php.ini file, or by adding this to the .htaccess file:

You can put it at the bottom.
There's no reason why it should have but you can simply remove it.
You can try editing the php.ini file instead.
Have you changed the post_max_size inp php.ini yet?
After you make the changes, restart Apache.
