In the \XF\Db\Schema\Column class, you have an after method for defining the AFTER _ SQL, but there is no equivalent for the FIRST statement, to add a column to the top of the table.

The absence of a feature isn’t necessarily a bug, it just indicates that at the time of implementation we had no requirement for it.

Probably useful though but we’ll just move to suggestions for now.
