Duplicate Image hidden in Spoiler shows in Latest Activity preview

People expect an image in a spoiler to be hidden. It may be adult or upsetting, a bit like this one:

Spoiler: Warning: contains scenes of graphic violence and nastyness

So it should not show a preview thumbnail in Latest Activity

at the pictures (hey, I'm evil) but yeah on your issue. Spoilered stuff should probably not be shown in Latest Activity. Kind of defeats the purpose, esp. in a case of gruesome brutal violence like this one.

Yeah, attachments in messages are a special case in the activity feed as they are shown regardless of whether or not they appear in the message body. You could remove them entirely with template edits if you really wanted to, but the context needed to hide attachments in spoilers is not readily available.
