How do I import from VB4?

Sorry I tried to do a search but I keep getting this:

Not a good sign

However, I have looked at the import feature and all I get is errors when trying to do an import from my current VB. Perhaps I do not know exactly how to put the information in the box.

Can someone post exactly where to find the information to plug in, and then an example of each field? I just got the donations from my guild to purchase this software, but want to make sure I can do an import first.
