At the purchase page for XenForo, it says the $140 license fee includes 12 months of ticket support and upgrades. Support and upgrade access can currently be extended for $40 for an additional 12 months (see details). Bulk discounts are also available. (details)

Does what they say mean support and upgrade access can only be extended for one year?

Support can be extended for as many consecutive years as you wish.
Most people just renew as the support expires.
