Just installed a fresh vanilla 2.1 and custom thread field location doesn't do anything. I create 3 custom fields each using the different location option and they all still display at the bottom of the main message box.
This is expected and has been the case since XF 2.0. When editing the values of custom fields, those fields will always appear below the rich text editor. The display location only relates to where on the thread they display:
