Create User Upgrade Blank(ADD-ON ISSUE)

So... we started with just this add-on:

All was well.

Then we needed more functions and installed this:

http://xenforo.com/community/resources/multiple-paypal-user-upgrades.1167/

When we would go to create a new upgrade, it would be glitched and would only show two text boxes.

We've gone ahead and removed that second add-on but now... our create user upgrade page is blank, I know this is caused via an add-on so no official support but curious if anyone has any ideas on where I could start looking for a problem or a fix.





I looked through the xf_user_upgrades table via phpmyadmin and didn't see any of the columns the second plugin uses, but... ya little stuck now.

File Health Check turns out fine, all other user upgrade functionality works.

We've attempted to contact the plugin authors with no luck.

Tried that, no go.

I disabled that first upgrades add-on and it actually works... So... not sure we'll try a fresh install of it or something, the developer seems MIA thanks for the idea borbole.
