Changes revert to original on a post in the moderation queue

When a post is edited that's in the moderation queue, after it's approved, it reverts back to its original state without the edits, but still retains the reason for edit.

I just moderated a post, edited it, approved it, and the edit was retained.

It's worth noting that there isn't actually a system that logs an edit reason. The only edit reason is related to sending moderator action alerts. Is this functionality from an add-on, are you able to reproduce it with all add-ons disabled?

*facepalm* Yeah, the edit reason is from an addon. I wonder if it's also causing the issue. Time for testing. I'll report back.
