Add-on Auto Populate New Thread

Hi guys

I've had a look, but think I am searching for the wrong thing...

Anyway, does anyone know of a script that when you create a new thread in a allocated forum/s it will have a pre-defined template filled out?

What I am wanting to do is have a system where staff and log things and when they create a new thread, it populates the text field with a template where they can fill in the values.

IE it will create a template like this:

First Name:​

Last Name:​

Address:​

Country:​

Email:​

Mobile:​

Home:​

Product:​

etc
