Add-on XB_Avatar User Name v1.0

Got this add-on to make users who haven't assigned a specific photo to their avatar to have their initial in a colored block.
Why do some members either show an avatar photo, or the “initial” the add-on created for us, while others have 2 names almost superimposed? User DCS1157 for example has his name kind of showing up twice in the current visitors screen, but if you look at his posts in his profile page, he has a big “D” there. J Clary same thing. Weird…..
