XF 1.5 Warning point n/a

One of mymods issued a third warning which is usually cause for a ban (3points). However when looking at the warnings on his end he sees the points as n/a. But on my end as admin I see 1 point. And there was no ban. I'm wondering what could cause this discrepancy?

Unless there is an add-on involved, the warning details are the same for all members with the relevant permissions to issue them.

Are you sure 'Apply warning points' wasn't unchecked when the warning was issued?

If it was would I be seeing 1 point as admin?
