its failed on my test forum, all given codes over phone failed, and my acc its locked.

And after @Chris D suggested

 $config['enableTfa'] = false;

here XF 1.5 - Two-Step Verification and Security Improvements got another problems with error.

And how far behind is the time on your device? That could have an impact.

Just checked, it is less than a second.

Just tried adding two-step verification on my site again, I entered the code within five seconds of it being generated (five seconds after it appeared, not before it was to expire), and it worked fine. I guess syncing a bunch of times corrected the error.
