XF 1.5 Registration issues after upgrade

Registration is broken after version upgrade from XenForo 1.3.5 to XenForo 1.5.13. No matter whatever I enter in the fields I always see following error message and my submission is not going through.

1. Please type a username that contains at least 3 characters.
2. Please type a functioning email address.
3. Please type a valid password.

I'm using custom template in front-end.

Outdated or invalid templates are the most likely reason.

Ensure you upgrade to the latest style if it is from a third party, otherwise merge/revert and outdated templates.

Thanks @Brogan, I found out that I was not using hashed field names in my template. Rather I was using plain field names, and that was the issue.
