There is a class on the links so you can style internal links as you wish.

In terms of getting the title, this has to be done very carefully as it's an information leak otherwise. (Leaking titles from private areas.)

What if it only kicked in if the friendly urls are on? (As the title will be in the url anyway - and it only add link text if none is supplied, or maybe manually adding a 'private' param in the url overrides /bypasses it.)
