save it, and upload it to your webhost where your site is.

eg:

rename the extension from .png to .ico (I don't think anything changed in that regard it's been years since I uploaded a favicon).

Still the same. Needs to be 16x16 pixels still as well.

Just use the site I posted a link to. You can upload the PNG you want to use, and it will create the full favicon.ico file for you to download and use.

That's because it's scaled up. The image HAS to be 16x16 pixels, which is why it's a dotted image.

It will give you a preview of what it looks like, as per the below screen shot
