How can I add a bbcode with an overlay?

I would like to add a custom bbcode to my editor. However, when users click on this button an overlay should appear with a form where they enter data. When they click on submit he overlay is closed and the parsed data is embed in the editor textarea.

Any ideas how can I do this?
