XF 1.2 Error reports (attachments)

I have noticed whenever I upload an image its broken until I submit the post the image displays fine. But I'm getting a bunch of error reports something to do with attachments. Can anyone shed any light on this? And how to resolve it.

I'd guess you have an add-on that's overriding this behavior and returning unexpectedly (thus not returning anything). Though what add-on, I'm really not sure.

I'll start disabling add-ons in trying to find what is causing it. It started on the 18th November and since then 102 pages of this error has been logged.
