Error Notifications auto-populating in Name and E-Mail fields while signing in

Hello There,

If we click "Sign Up" button, then in the pop-up select radio option "No, create an account now" and then click on "Sign-Up" and in the form enter the values for Username and E-Mail ID, then the fields showing the error. Please find the screenshot attached:

However, this is likely related to an add-on or custom style. Please confirm with all add-ons disabled and on the default style.
