How often do they do one on you? Yearly?
Have you ever had a really good one? bad one? One that you thought was unfair?

The last two companies I worked at had annual reviews. I thought the reviews were fairly accurate. In both companies, the manager completed the review and that manager's supervisor had to review it and sign off on the review.
