Current best implementation of Adsense

Is the current best practice to put the single script in PAGE_CONTAINER and let Google manage display of ads? I get a very weird implementation that way, and CPMs are abysmal. Thanks

Do you guys block any networks or categories of ads?

You used to be able to see what categories were performing best for you, can seem to find that now.
