Any functioning crypto payment add-on will upgrade a user once payment is confirmed, assuming the webhook sent by the payment processor arrives, contains the correct information and is processed properly.
The problem is mostly with recurring upgrades (subscriptions), where a payment should be auto-deducted every period so as to maintain the user upgrade. This is not possible on some blockchains (e.g. Bitcoin where there are no smart contracts) so in those cases the payment processor only sends the user a reminder that his payment is due.
