Fixed bad redirect location following "password reset" and then login

Visit the Forgot your password page. After submitting the request you are taken to this page:

index.php?lost-password/lost



Now use the login bar on this page to successfully login. You are redirected back to the same page with an error:

index.php?lost-password/lost

I've adjusted some of these pages to simply redirect you to the index if you're already logged in.
