XF 2.1 Avatar & Signature Image Issues

I am running 2.1.3 and I am having an issue where uploading a new avatar image basically does nothing. It will not let you do it. No error message pops up, the screen just flashes and then that's it. The same thing is happening when trying to use a URL image in signatures. The image flashes on the screen for a second and then it's gone. This is also happening with smilies in signature too. They flash on the screen and then disappear.

Someone suggested it might be a bug in 2.1.3 and to update to 2.1.4, which I did on my test site, but the issues are still there.

Any idea what is going on here?

Check the User group permissions for the Registered group:

The permission is set to Yes.

Check this:

Admin control panel -> Groups & permissions -> Analyze permissions -> User name -> Analyze -> Signature permissions -> Allow images

It's still all set to yes.
