XF 2.1 Approval Queue Doesn't Work

Hello,
This feature "Approval Queue" does not work in our forums. We upgraded from v1.5 to 2.0.8 and then to 2.1.2 but since the upgrade to version 2.0, the Approval Queue would not work. Whatever actions moderators choose, the queue won't get shorter and the item still remains in the list (queue). What should we do about this problem? We don't know where to check...

One of the few threads I have found, but having the same issue.

Ever since I re-installed the forums, all actions I do in the approval queue only refreshes the page and does nothing. I am forced to manually go to the profile and tag the "spam" option to ban them manually.
