I'm using ******* - Credits 1.7.1
I can post my template if you want? But i don't know which template you want to see so yeah :/
 
Never used that add-on so I have absolutely no idea what the template names would be or even how it is displayed.

I would suggest asking the developer for support, sorry I can't help beyond what I am saying.
