XF 2.3 Alert on mentions not working properly

In our forum, some users have reported that when someone else mentions them (@Username), sometimes it's not showing in the alerts. This used to be a pretty stable feature, but it's not working 100% nowadays.

Any idea on how to check if anything has changed in the admin panel? The profiles are to receive notification on mentions, so it's not a problem on their end.

One thing about alerts is that if it is added after a post is submitted it will not alert the @ username.
Example, submit a post then edit the post and add the @ username = no alert.

That's something we noticed during our tests.

We also noticed that if the user is seeing the topic, sometimes the notification doesn't show up.

Sometimes it doesn't show even if the user is not seeing any topic. We still didn't figure out what exactly is the issue.

As far as I know, it is happening to everyone, but only a few more hardcore users have noticed and mentioned it.

We noticed it a couple of weeks ago. We even updated the xenforo (it was version .5 or .6), but it's still happening.

Maybe there are a few limitations and cases where mentions wouldn't work (like editing), but we are not sure what those are.
