The same way as you did when you originally installed the add-on. It's the same routine. Make sure advapp_install.php is uploaded to your server, run it and enter your order number.

Thanks, for some reason I can't seem to download it from your site. I only see an option to purchase. My screen name is the same there.

There's nothing to download. Use the SAME advapp_install file that you received when you first purchase the add-on.
From the README.txt file you received when you downloaded the add-on...

I am back, let me rephrase:
When a user submits a form he gets a forum threat where he can respond to any question forms the community (logged-in) or provide more info about them self.
Second there is a threat that has the same info that the submitter submitted but is not visible to the submitter only to the community, so the community can discuss the submitter his application.
This can be done with 2 threats one visible to the submitter and one visible to the community (logged-in).
If you would provide and second option to post the New Thread (http://imgur.com/ed6GjlT), so there are 2 place where the thread is places, I can then change the forum permissions to the correct values.
The forum url is Noxcrew.com (If you want to see what kind off community it is)

Works very nicely.
1 Bug: under "User Redirect Options" the "Newly Created Thread" option goes to the second thread not the first thread.

I located the initial cause of the error, but I'm not sure why I didn't catch that with my install tests.
I'm going to start a PC with you with instructions on what needs to be done in a few minutes.

No updates are sent by email. Just run advapp_install.php again to install the latest release.
Also, be sure to read the PC I started with you for instructions on how to recover from the error you received.
For everyone else's benefit, the error Kingston received would only happen on new installs. It would not happen with updated installs.
