Yes. XenForo has a trophy and points system built in.
I strongly recommend testing it out with the Admin Demo: http://xenforo.com/demo/
If you click on my username next to this post, you will see my trophy points. Also, you can then click on the number of trophy points to see the trophies I have achieved

It should be possible to hide the numbers if you don't want them at all... but this following template modification adds icons next to the numbers:

Built in there is the ability to show the number of trophy points below people's usernames. Switch to the "Kitchen Sink" style: http://xenforo.com/community/misc/s...=/community/threads/achievements-addon.41980/
You'll notice that you can add quite a lot of information to below the username, including the number of trophy points.
There is a third party add-on which can replace the trophy system with medals. You can use this to display the individual medals each user has achieved. You can see a preview of how that looks here: http://xenforo.com/community/threads/trophies-in-mini-profile-user-info-pane.34245/#post-433721

With the third party addon will you have to add them manually to each person?
The one built in Xenforo can you display them under the username because need the trophies icons instead of numbers.
I'm not sure how the medals system works. You could send a message to its developer (xfrocks) who may be able to answer in more detail.
It isn't possible to do without customsiation. It would most likely need an add-on creating for it. By default it will only display the trophy points with a link to the medals.

Here is what xfrocks said: "The add-on will allows you to manage medals, categories. Admin has to give out medals manually. However, there are 2 extensions (paid) for this add-on, you can automatically award medal using trophy data and user group data."
