Is your guest browsing cached? That reduces a lot of load in itself too. I think there are means to optimize it some what but ,with nginx , I have zero experience. Block those ip and ranges too. Block any agents that follow a pattern but guest caching reduces the load off mysql which hammers...

Are there any specifics in terms of what is being accessed? like URL?=Varient or similar? If the main brunt is one geo, I would block that GEO temp until it slows down. Find a common factor used due to the attack.

Why? Usergroup permissions also allow and disallow attachment access.

Just set group permissions where X team members can access content shared by Y. Y can only be viewed by thread starter and assigned team of X. Simple.
