I have done something wrong, but can't figure it out. I've got it installed OK, but nobody can see any options to change their Username. I tried myself with a test account and can confirm that it's not showing anywhere under Your Profile, Preferences or Account Details. I think I missed a step...

No go. While it doesn't give me any errors, nothing displays on the page where I placed it (or elsewhere).

I setup some custom fields and one is a Yes/No Radio option, but it also displays a No Selection which is selected by default. How can I remove this option and select either Yes or No by default?
