Who can help prevent spam signups and some slight design changes when needed?

Lately we have been getting a lot of spam sign ups and need help blocking this, I have honeypot, all the xenforo settings, google captcha and Q&A - still getting thru like crazy and posting. I even have AntiSpam by cleantalk, which seems like a joke. I am looking for a real professional company to help with this.

I am also looking for some slight design changes when needed.
